Southwest General Health Center notified more than 480 patients who took part in an obstetrics study that a file containing their private information was missing, according to a local news report.

The file missing since December 5 contains information that was collected from April through October 2013.

It includes patient names, dates of birth, medical history numbers and clinical data. Southwest General said the file did not contain Social Security numbersor financial information.

The hospital tried to locate the missing file. However, it is unable to find it.

It also apologized to its patients and stated that they have implemented some procedures to prevent a similar incident in the future.
